This cosmetic eyelid procedure is used to remove cholesterol-filled skin lesions called Xanthelasma that appear around the eyelids.

What is Xanthelasma?

Xanthelasma (xanthelasma palpebrarum) are small, flat or slightly raised yellowish deposits of cholesterol that develop under the thin skin of the upper or lower eyelids, often near the inner corner of the eye. They tend to be benign, but can grow over time.

Although Xanthelasma itself is not dangerous, it can sometimes be linked with high cholesterol and lipid imbalances, so a Health Assessment is recommended.

Is Xanthelasma surgery right for me?

You may choose Xanthelasma surgery if you have:

  • Persistent yellow eyelid bumps
  • Cosmetic concerns about appearance
  • Lesions that grow or affect your confidence
  • A desire for a smoother eyelid contour and skin texture.

Because Xanthelasma won’t naturally go away, surgical removal can be an effective option when lifestyle changes alone don’t reduce the deposits.

What happens during Xanthelasma surgery?

Preparing for Xanthelasma surgery

Your Nuffield Health consultant (surgeon) will carry out a detailed examination and review your medical history before deciding if Xanthelasma surgery is suitable for you.

During your operation

Xanthelasma surgery is typically a minor outpatient procedure carried out under local anaesthetic :

  • Your consultant (surgeon) carefully removes the cholesterol deposits from the eyelid skin
  • For some patients, laser treatment or chemical methods may be used depending on the size and location of the lesions
  • The aim is to remove the deposit while preserving the natural eyelid contour and function.

The choice of technique — whether surgical excision , laser ablation or cryotherapy, depends on your individual needs and your Consultant (surgeon’s) recommendation.

What to expect after Xanthelasma surgery?

Recovery from Xanthelasma surgery is usually quick and straightforward:

  • Mild redness, swelling or tenderness may occur for several days
  • Small scabs or crusting are normal and typically heal within 7–10 days
  • Most people return to everyday activities soon after the procedure.

Your consultant will provide you with clear guidance on pain management, wound care, sun protection and when it’s safe to resume skincare or makeup around the eyelids.

What are the benefits of Xanthelasma surgery?

Xanthelasma removal can:

  • Improve eyelid appearance and symmetry
  • Reduce the visibility of yellow cholesterol deposits
  • Restore confidence and cosmetic balance
  • Offer a long-term solution.

What are the risks and complications of Xanthelasma surgery?

As with all eyelid procedures, there are some risks to consider:

  • Slight scarring or pigment changes
  • Temporary swelling or redness
  • Risks related to anaesthesia or wound healing
  • Recurring Xanthelasma, especially if your cholesterol remains high.

Before your surgery, your consultant (surgeon) will talk you through these risks, answer any questions, and help you feel more confident going into surgery.
