To get started with a patient referral, consult the eligibility criteria below and follow the instructions at the bottom of the page.

Our Rehabilitation Specialists support participants living with a range of conditions including Cardiovascular Diseases, Respiratory Conditions, Rheumatological Conditions, Metabolic Disorders, Musculoskeletal Conditions, Neurological Disorders, and Long COVID. If they are eligible, we will contact them to arrange the next steps.

Once you refer a patient, they will receive a simple questionnaire, via email, to ensure the programme is suitable for them.


  • Eligibility criteria
    • Must be 18 years of age or over
    • Walk with or without walking aids a minimum of 20 metres
    • Independent wheelchair user
    • Can exercise without the use of an oxygen tank
    • Has not had any surgery in the last 12 weeks and has been discharged from any post-operative rehabilitation/care
    • No planned surgery in the next 24 weeks
    • Is not pregnant
    • Must be able to communicate verbally in English and be able to read English
    • Must be ready to commit to participate in a 12-week rehabilitation programme
    • Must be able to travel to the selected site, twice per week, for 12 consecutive weeks.
  • Exclusion criteria
    • Unstable angina
    • Uncontrolled cardiac arrhythmias
    • Severe aortic stenosis
    • Uncontrolled symptomatic heart failure
    • Acute myocarditis or pericarditis
    • Acute or recent myocardial infarction (MI)
    • Acute pulmonary embolism (PE) or deep vein thrombosis (DVT)
    • Uncontrolled hypertension
    • Severe symptomatic aortic dissection or aneurysm
    • Infective endocarditis
    • Acute systemic illness or fever.


About our Rehabilitation Specialists

Our Rehabilitation Specialists are exercise professionals who have completed additional CPD training to support patients with long-term health and complex health needs.

Once enrolled, they’ll create a personalised exercise plan across gym, community, and healthcare settings, using evidence-based techniques to help them manage symptoms and build healthy habits more independently.

Working closely with healthcare professionals and multidisciplinary teams, they are here to help your patient move more easily, feel more confident, and enjoy greater independence and wellbeing through safe, effective exercise.

Qualifications and Competencies

Our Rehabilitation Specialists are trained to assess your patient's overall health, personalise exercises to suit their needs, and use behaviour change techniques to support them in managing their condition.

They work with patient's living with a wide range of health conditions, including heart and lung problems (cardiorespiratory), joint or muscle issues (musculoskeletal), neurological conditions, diabetes (endocrine), and cognitive health conditions.

The Long-Term Conditions programme operates within Nuffield Health's clinical governance framework, ensuring our Rehabilitation Specialists work alongside healthcare professionals and multidisciplinary teams, to make sure they get safe and clinically backed support at every stage of the programme.

We’ll check your patient's eligibility and guide them through a clear, structured pathway to help them get the most from the programme.


How to refer a patient

  1. Enter your patient’s postcode to find their nearest Nuffield Health Fitness & Wellbeing Club or community venue.
  2. Review the available locations and select 'Submit a referral' for your patient's preferred site.
  3. Complete and submit the form. Patient email address must be correct to progress application.
