What is our Long-Term Conditions Programme?

We believe everyone deserves a healthy, happy life. That's why we've launched the Nuffield Health Long-Term Conditions Programme.

It's a free 24-week structured exercise programme offering specialist advice from a Rehabilitation Specialist about the benefits of movement for people living with long-term symptoms like chronic pain, breathlessness, and long-term fatigue.


How does the programme work?

Our clinically trained Rehabilitation Specialists will develop a tailored exercise programme, helping you manage your symptoms and make gradual, sustainable improvements to your health and wellbeing.


Who delivers the programme?

Our Rehabilitation Specialists are exercise professionals with specialist training who support people like you living with long-term health conditions and complex health needs.


Once you’re enrolled, they’ll create a unique and personalised exercise plan to help you manage your symptoms and build healthy habits.

Working closely with healthcare professionals and multidisciplinary teams, they are here to help you move more easily, feel more confident, and enjoy greater independence and wellbeing through safe, effective exercise.

FREE gym membership for 24 weeks
You’ll have full access to select Nuffield Health Fitness & Wellbeing Clubs and Nuffield Health 24/7, where you’ll find a full range of online workout videos throughout the programme.


What’s involved?

Our Rehabilitation Specialists will guide you through safe, simple movements that fit your needs. We’ll help you understand how movement can improve your health and get you back to doing what you enjoy most.

Here’s what you can expect from the Long-Term Conditions Programme:

Weeks 1 to 12

Two one-hour group sessions per week focusing on tailored movement and health coaching. Health coaching involves group-related conversations to build confidence, motivation and support you to reach your goals.

Weeks 13 to 24

During weeks 13 to 24, you will continue to exercise independently and attend a weekly supervised drop-in session with your Rehabilitation Specialist.

Throughout your journey, you will also have FREE assessments to monitor your progress.


How do I apply?

Once you’ve spoken to your GP or healthcare professional about the benefits of the programme, getting a referral is quick and easy.

You can be referred by your:

  • GP
  • Doctor
  • Nurse
  • Pharmacist
  • Social prescriber
  • Physiotherapist
  • Care navigator
  • Health and wellbeing coach

Simply share this page with them and ask them to complete a referral on your behalf. They will need to provide a valid email address for you so we can get in touch.

If you do not currently have a healthcare professional who can refer you, you can submit an enquiry form. A Nuffield Health clinician will then contact you to assess your suitability for the programme and, where appropriate, make a referral.

What happens next?

Once we’ve got your referral, you’ll receive a simple questionnaire via email to make sure the programme is suitable for you.

We'll make you an appointment for an assessment with your Rehabilitation Specialist to make sure you're ready to get started.

If you're joining the programme at a Nuffield Health Fitness and Wellbeing Centre, this will take place at your selected centre.

If you're joining the programme in a community venue, your assessment will take place over the phone.
