To give you the help you deserve, we’ve put together a completely FREE programme that’s available to everyone.

Who is the programme for?

Our unique tailored movement programme is designed to help people living with one or more long-term conditions who are suffering from the following symptoms:

  • Chronic pain
  • Long-term fatigue
  • Breathlessness


Why join the programme?

  • It’s completely FREE.
  • You’ll receive one-to-one guidance from our Rehabilitation Specialists (who are Clinical Exercise Specialists) and a tailored movement programme.
  • We’ll provide support to help you progress towards moving independently.
  • You’ll also be able to use your selected Nuffield Health fitness and wellbeing centre FREE for 24 weeks while on the programme.

Our team will support you with simple activities you can do in the gym and at home. You’ll also get personalised advice to help you feel more confident and in control of your health moving forward.


What’s involved?

Our Rehabilitation Specialists will support you with simple movements that are safe and suited to your unique needs. You’ll also get advice to help you feel more confident and in control of your health.

Weeks 1–12

Two one-hour group sessions per week focusing on tailored movement and health coaching.

Weeks 12–24

You will continue to exercise independently and attend a weekly class with your Rehabilitation Specialist on the gym floor.

Throughout your journey you will also have FREE health checks to monitor your progress.

How do I apply?

You need a referral from a healthcare professional to get started.

You can be referred by your:

  • GP
  • Doctor
  • Nurse
  • Pharmacist
  • Social prescriber
  • Physiotherapist
  • Care navigator
  • Health and wellbeing coach

Just share this page with them and ask them to refer you!

Once we’ve got your referral, we’ll give you a call and ask some simple questions to make sure the programme is suitable for you before you get started.

We’ll make you an appointment at your selected Nuffield Health fitness and wellbeing centre for a health check and assessment with your Rehabilitation Specialist to make sure you’re ready to get started.


Information for healthcare professionals

Our Rehabilitation Specialists are Clinical Exercise Specialists with enhanced clinical training who work within multidisciplinary teams.

Through an Extended Scope of Practice model, they prescribe exercise to people with chronic conditions, effectively bridging the gap between clinical care and community support.

Our Rehabilitation Specialists support participants affected by a range of conditions which include Cardiovascular Diseases, Respiratory Conditions, Metabolic Disorders, Musculoskeletal Conditions, Neurological Disorders, and other conditions such as Long COVID.

Once you refer a patient onto our programme, we will call them to ensure the programme is suitable for them and book them in for an initial assessment and health check.


  • Eligibility criteria
    • Must be 18 years of age or over
    • Walk with or without walking aids a minimum of 20 metres
    • Independent wheelchair user
    • Can exercise without the use of an oxygen tank
    • Has not had any surgery in the last 12 weeks and has been discharged from any post-operative rehabilitation/care
    • No planned surgery in the next 24 weeks
    • Is not pregnant
    • Must be able to communicate verbally in English and be able to read English
    • Must be ready to commit to participate in a 12-week rehabilitation programme
    • Must be able to travel to the Nuffield Health & Wellbeing centre, twice per week, for 12 consecutive weeks.
  • Exclusion criteria
    • Unstable angina
    • Uncontrolled cardiac arrhythmias
    • Severe aortic stenosis
    • Uncontrolled symptomatic heart failure
    • Acute myocarditis or pericarditis
    • Acute or recent myocardial infarction (MI)
    • Acute pulmonary embolism (PE) or deep vein thrombosis (DVT)
    • Uncontrolled hypertension
    • Severe symptomatic aortic dissection or aneurysm
    • Infective endocarditis
    • Acute systemic illness or fever.
